NOTICE OF PRIVACY PRACTICES

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.


At Play Connections LLC, we are committed to protecting the privacy of your child’s health information. This Notice describes our legal duties and privacy practices regarding Protected Health Information (PHI).
 

1. OUR PLEDGE REGARDING HEALTH INFORMATION

 

We create a record of the care and services your child receives at Play Connections LLC. We need this record to provide quality care and to comply with legal requirements. We are required by law to:

  • Ensure that PHI that identifies you or your child is kept private.

  • Provide you with this Notice of our legal duties and privacy practices.

  • Notify you following a breach of unsecured PHI.

  • Follow the terms of the Notice currently in effect.

2. HOW WE MAY USE AND DISCLOSE PHI

 

We may use and disclose your child’s PHI for the following purposes:

  • For Treatment: We may use PHI to provide occupational therapy services. We may disclose PHI to doctors, teachers, or other healthcare professionals involved in your child's care to coordinate treatment.

  • For Payment: We may use and disclose PHI so that the services may be billed to and payment collected from you, an insurance company, or a third party.

  • For Healthcare Operations: We may use PHI to run our practice, improve your child’s care, and contact you when necessary.

  • Appointment Reminders: We may use and disclose PHI to contact you as a reminder that your child has an appointment for treatment.

3. SPECIAL CIRCUMSTANCES (WITHOUT AUTHORIZATION)

 

Federal and California law allow us to share PHI without your authorization in these situations:

  • Required by Law: When federal, state, or local law requires it.

  • Public Health & Safety: To prevent a serious threat to the health or safety of your child, the public, or another person.

  • Abuse or Neglect: We are mandated reporters in California and must disclose PHI to report suspected child abuse or neglect.

  • Lawsuits and Disputes: In response to a court or administrative order, or a subpoena.

4. USES THAT REQUIRE YOUR WRITTEN AUTHORIZATION

 

The following uses and disclosures will be made only with your written authorization:

  • Marketing Purposes: We will not use PHI for marketing without your permission.

  • Sale of PHI: We do not sell PHI.

  • Psychotherapy Notes: Most uses and disclosures of psychotherapy notes (if applicable) require your authorization.

  • Substance Use Disorder (SUD) Records: Per 2026 updates (42 CFR Part 2), any SUD records we maintain have heightened protections and cannot be used in criminal/civil proceedings against the patient without specific consent or a specialized court order.

5. YOUR RIGHTS REGARDING PHI

 

Right to Inspect and Copy: You have the right to inspect and receive a copy of your child’s health and billing records.

  • Right to Amend: If you feel the PHI we have is incorrect or incomplete, you may ask us to amend the information.

  • Right to an Accounting of Disclosures: You can request a list of certain disclosures we made of your PHI for purposes other than treatment, payment, or operations.

  • Right to Request Restrictions: You have the right to request a limit on the PHI we use or disclose. Note: If you pay for a service "out-of-pocket" in full, you have the right to demand that we do not share info about that service with your health insurer.

  • Right to Confidential Communications: You can ask that we contact you in a specific way (e.g., home vs. office phone) or at a specific address.

6. CHANGES TO THIS NOTICE

 

We reserve the right to change this Notice. We will post the new Notice on our website (www.theplayconnections.com) and have copies available at our clinic.
 

7. COMPLAINTS

 

If you believe your privacy rights have been violated, you may file a complaint with:

You will not be penalized or retaliated against for filing a complaint.
